UC Riverside Softball Hosts Open Tryouts On Saturday, October 13 At 9 AM

Riverside—UC Riverside Head Softball Coach Linda Garza announced today the Highlanders are hosting open tryouts for the 2013 season on Saturday, October 13 from 9 am - 12 pm at Amy S. Harrison Field.
The softball coaching staff will be evaluating interested athletes on skills related to offense, defense, and pitching (if applicable). A conditioning test will also be administered following the conclusion of the tryout. Athletes are required to bring a bat, glove, and cleats.
The Highlanders will practice from 8 am - 12 pm Monday through Saturday during the fall quarter. Softball players take classes from noon on, and all members of the team are required to be present at practice and to schedule academics according.
Softball Tryout Procedures
All of the following must be completed by October 10, 2012 in order to participate in the October 13, 2012 Softball Tryout. No exceptions will be made.
Step One: Medical Procedures:
All tryout participants will need to completely fill out and turn in the below Medical items to:

o Insurance Benefits Letter of Understanding
o Emergency Contact Info Form
o Optical/Dental Insurance Page
o Concussion Info & Agreement Form
o Medical History Form
o Pre-Participation Physical Evaluation
o Prescription Medications, Supplements and NCAA banned Drugs
• A Front and Back copy of your current medical insurance card
• Fill out and fax to the California Department of Public Health the new Student CA born sickle cell form (click to download):
• Turn in copy of the Fax/Receipt of Sickle Cell form to the UCR Sports Medicine Dept.
Step Two: Try Out Form Checklist:
1. Fill out and complete Waiver of Liability Form
2. Fill Out Try out Form Checklist with: Name, UCR ID, Sport
3. Get signature for Medical Approval from Christina Orozco confirming all medical forms have been completed
4. Turn in Tryout Form Checklist and Waiver Liability Form to:

Upon completion your form will be processed by Compliance and you will receive a confirmation from:
